Your Specific Responsibilities:
Volunteer Provider Program (VPP) Coordination
Coordination of the VPP for Citrus County in consultation with the region 4 volunteer program coordinator.
Submits quarterly and annual report of volunteer time and program deliverables as required.
Engage with local medical community to recruit volunteer health care providers.
Interact frequently with registered volunteers.
Determines participant eligibility based on current guidelines
Refers community members to needed health services.
Case management of clients referred to the program, conducting eligibility interviews, scheduling referral appointment with specialists, and follow-up on appointments.
Maintains updated program client records with plans, notes, appropriate forms, or related information.
Contacts clients in person, by phone, or in writing to ensure that they have completed required or recommended actions.
Conducts quality assurance programs to monitor the effectiveness of the program and its partners as pertains to state requirements.
Daily use of computer skills including but not limited to Word, Outlook, and Excel. Working with computers to enter data, or process information.
Dental Program
Provides support to dental programs by entering and billing services and providing administrative support.
Travels to schools and other offsite locations with the dental program and assists with transport of supplies and equipment, provision of services, and oral health education.
Provides education to students regarding improving general oral health.
Enters SEALS data per program requirements.
Assists with inventory control and management for dental program.
Support the school health program with data entry into HMS and SEALS.
Assist families in locating and accessing their assigned dental care provider.
Performs physical activities that require considerable use of arms, legs, and moving the whole body, such as climbing, lifting, balancing, walking, stooping, and handling supplies, materials and portable equipment
Swim Voucher Program (SVP)
Serves as the primary coordinator for SVP
Monitors application portal
Determines participant eligibility based on current guidelines
Issues vouchers
Submits reports as required by SVP
Promotion of Citrus CHD programs
Represents Florida Department of Health Citrus at community events and meetings including but not limited to workshops, and health fairs, Focusing on VPP
Communicates with people outside the organization representing the organization to customers, the public, the local medical community. This information can be exchanged in person, in writing, or by telephone or e-mail.
Distributes flyers, brochures, or other informational or educational documents to inform members of a targeted community.
Advise clients or community groups on issues related to self-care, such as improved oral hygiene and access to medical care
Time sheet and EARS entry within the required time frame
Completion of all mandatory or assigned trainings within the required time frame.
Attends scheduled trainings as required.
Performs other work as assigned.

Ability to maintain client records. Ability to understand and apply the methods and protocols used in maternal and child health. Ability to interview clients to identify needs. Ability to provide information and referral services. Knowledge of home visiting. Ability to plan, organize and coordinate work activities. Knowledge of community resources. Knowledge of the principles and techniques of effective verbal and written communication. Ability to actively listen to others. Knowledge of personal computers and software.
Qualifications:
