Your Specific Responsibilities:
This position serves as the Area 12, Volusia and Flagler County program designed to link racial and ethnic minorities to HIV Care and services using the Antiretroviral Treatment and Access to Services (ARTAS) and FDOH Linkage operating processes for engagement and retention in HIV care and treatment. The role of the Area 12 ARTAS Linkage Coordinator is to ensure individuals with HIV infection or an AIDS diagnosis, including pregnant women, are linked to and provided rapid access to treatment to ensure retention in HIV Care as part of Florida’s plan to eliminate HIV transmission and reduce HIV related deaths. ARTAS work with newly identified HIV positive and out of care HIV+ individuals to remove potential or present barriers and assist with navigation of HIV services and care to support engagement and retention in their HIV care. Performing case management activities. This position may also work to link the HIV negative individuals or partners of clients to facilitate initial Pre-Exposure Prophylaxis (PrEP) Services navigation. Conduct field visits to locate individuals infected with or suspected to be at-risk of infections. STD counseling and testing, medical evaluation and /or treatment. HIV testing, HIV pre-and post testing counseling, routine testing at the local county jails, visual case analysis and epidemiological techniques, data entry and/or other related activities. Data entry into STARS, HIV/AID Linkage Module, CareWare and HMS systems. Provide education to clients. Communicates with and visits public and private clinics, clinicians, hospitals, laboratories and other health care providers. Provide technical assistance not considered part of routine duties including but not limited to: Epidemiological assistance to other CHDs or program areas, public health awareness and emaegency response situations. Collect and analyze data obtained from pulling electronic labs, manual data entry, pulling faxes and distributing to apprppriate staff.

Required Knowledge, Skills, and Abilities:
Ability to use computer and computer software(Word, Excel and Powerpoint).
Ability to review and access client files.
Ability to develop methods to monitor & evaluate qyality of service and compliance with rules, polucies and statutes.
Qualifications:
Minimum -

Must have HIV and STD experience
Must have a valid Driver's License
Must have more than 1 year of case management experience
Must be a current Florida Department of Health employee, in a Selected Exempt Service (SES), Career Service (CS), or Other Personal Services (OPS) position
Preferred -
Ability to establish and maintain liasion with other agancies.
Ability to develop manuals, policies, procedures, standards and rules.
